♦ As you’re reading this, find a large envelope, TODAY. LABEL IT “2009 TAX INFORMATION” AND TAPE IT BY YOUR PHONE IN YOUR KITCHEN OR YOUR BEDROOM. Start putting anything tax related in it. If you have doubts as to whether or not its tax related, put it in anyway because most likely it does. Let us figure it out for you later. If you don’t do this now, you won’t remember where your papers are later. ♦

  • The 5 Most Frequently Forgotten Items that could hold up your return:

  • DMV registrations.
  • All home mortgage statements (a loan company can transfer your loan to another company and you only bring me one statement).
  • Property Taxes paid.
  • Final escrow closing cost (Hud-1) statements for those that have refinanced, bought or sold property.
  • Your December 2008 & January 2009 bank statements for those that earn less than $ 10 interest. (Banks are not required to send you a statement if less than $ 10; but your are required to report that. Look on the back of my card for more helpful hints!!

Instructions/Check List for Mail-in & Drop Off Clients:

  • For New clients or clients with dependent changes, please provide: copy of the Social Security Card (or write down the name & number exactly as it appears), Home Address, Filing Status (married, single, married filing separate, etc.), Date of Birth, Occupation(s), Contact Phone Numbers (Home or Cell) & E-mail address.
  • For returning clients or clients with no changes to any information from last year, just list the dependents & include your home and cell phone number with e-mail address so we can contact you easily.
  • All New Clients must send me a copy of your Income Tax Returns from last year if you filed a Long Form.
  • Send ALL portions of your W-2’s (will be either 2 or 3) except for (1) copy that says “Employee Copy”
  • Send ALL 1098-Mortgage Interest forms: Interest you paid on your home from each bank
  • A Copy of your Property Tax Statement or write down the amount of Property Taxes you paid last year.
  • If Refinanced or Bought a home last yr, send a copy of the Final HUD-1 Closing Cost Statement.
  • Send ALL 1099-INTEREST, 1099-DIVIDEND & Stocks & Bonds (Barter & Exchange) Forms.
    * If you didn’t receive a form, but have interest to report, please send me (or write down) the amount found on bank statements in either the previous year’s December or current year’s January statement.
    * If you don’t have any interest to report, then please write down: NO INTEREST LAST YEAR.
  • Send any 1099-G Unemployment Forms, W2-G Gambling Forms, 1099-R Distribution Forms.
  • Write down amounts paid for individual DMV Registration.
  • Any Additional Expenses write on separate piece of paper, the category/name of the expense and the amount paid for each expense plus the amount paid in the previous year. Examples: Union Dues, Charitable Contributions, Safe Deposit Box, Tax Prep Fees, Professional Journals, Protective Uniform/Work Clothing, Laundry, Cleaning, Work Tools, Cellular Phone Bills, Continuing Education and Training Expenses as well as anything else you feel related to your work. (I will go over it later with you). Please see the back of my card for additional items I need.
  • Send check in the amount of $ 100.00. This deposit is to start the process of your Income Tax Return. It will also serve as the account information needed where your Refund Check will be directly deposited.
    *Make check payable to John Vartanian or Independent Tax, Real Estate & Mortgage Services, Inc.
  • For those who are Self-Employed, send ALL 1099-Misc forms. If you don’t have any, let me know the amount you made last year (This can be figured out by adding up all the deposits in your business bank account for the year). List your occupation. *Business deductions can be written down or better yet; copy your last year’s deductions from your Schedule C, and write down the amounts for this year next to last year’s amount.
    *Please include: Year, Make, and Model of the car used in the Business; the total miles driven for the entire year for all purposes and of that total, the amount of miles driven for business purposes.
  • Those with RENTAL Properties, write down income received, mortgage interest & property taxes paid, home owners insurance and amount for repairs. If a new rental, I need the property tax statement & date rented.


This is what will happen next in the Income Tax Preparing Process:

1. The day we receive your package, we will call you to confirm receipt.
2. We will call you again if there is any additional information needed.
3. John will personally call you & go over your return in detail making any changes necessary. You will be informed of your refund(s) & if all is acceptable, the return will be processed.

  • Closing Thoughts:

    I want to thank all of you that have referred your family and friends over the years as well as your future referrals. This is the highest compliment I can receive from you. This shows me how much trust you have in our services. With respect to that trust, I want to ensure you all that I do not, have not, and will not ever share any of your personal or private information to anyone outside this office. Once your personal information has been transferred to my computer, I am the only one with access to it from that point on.

    Now you should be ready to call for your appointment. If you have any additional questions, please leave a detailed message with either our receptionist(s) or office manager(s) and I will get back with you. As most of you already know, I am usually working very late during the tax season to better service you. So please understand that I do not consider it inappropriate to return your call as late as 10:30 pm or 11:00 pm to review YOUR Income Tax Return with you. If that hour is to late for you, please indicate the best time to reach you. I can be reached as late as 3:00 am any day of the week during Income Tax Season.
